There are marketers who use it multiple times in one page as well. As long as it does not clutter or interfere with the visibility of the page, use it.
Are you using your social media pages: You must integrate your favorite social media platform with your opt-in form or vice a versa. It is a valuable source of traffic that cannot be ignored. If you are regularly updating your social media page then you will never find a dearth of traffic from it.
The Easier the better: The more information you request, the fewer people will opt-in. In most cases, a name and an email address should suffice. If it's not necessary, don't include it here.
